Surgical Removal of a Huge Epiglottic Lipoma: Case Report

Background: Lipoma is a very rare benign tumour of upper aero-digestive tract with less than 115 cases described in the English-literature. They account for approximately 1% tumours larynx and oro/hypopharynx. The symptoms are variable including progressive horseness, dysphagia even severe dyspnea which can sometimes be life-threatening. clinical presentation lipoma important particularly during induction general anesthesia, they cause unpredictable airway obstruction. Surgery treatment choice includes endoscopic techniques, microscopic laryngeal surgery external surgical approach (cervicotomy). However, standard management large lipomas epiglottis has not been present yet. In this article, we case report huge successfully treated tracheotomy approach—cervicotomy hyoidthyroidpexy without sacrificing any structure. Case Presentation: We 54-year-old female on lingual surface that extends upwards to level left aryepiglottic fold narrowing pyriform sinus, making impossible our anesthesiologist glottic visualization orotracheal intubation. Following tracheotomy, was inadequate manipulate epiglottic lipoma. Instead, performed macroscopic (cervicotomy hyoidthyroidpexy) pulled into endolaryngeal window forceps then dissected from surrounding tissues “in toto”. Conclusion: Despite benign, because being potential different procedures able manage it. after enabled extirpated leaving remnants or causing excessive damage.
